Full Access to Your Doctor
Literally. After hours, weekends, holidays – there’s no bad time to receive excellent medical care. Easily schedule your appointments by phone or online.
Extended, Relaxed Visits
You won’t feel rushed through an appointment, and there’s never a question you won’t have time to ask. Part of the relationship you’ll build with your doctor is based on your understanding of your care. Now that’s what we call personal healthcare.
Same-Day and Next-Day Service Scheduling
Because Dr. Hoffman reserves time for a select number of patients at the practice, there will be no “squeezing you in.” You’re a priority, and you’ll feel like one.
Wholesale labs, radiology, and medication costs
Our savings are extended to you. All prices will be quoted before your labs are ordered or your prescription is filled.
Full Access Via Technology
You’ll be able to reach us and your medical information via internet, e-mail and text.
A Comprehensive Annual Physical and Employment/School Physicals
This exam will be fitted to your personal medical needs. It’s all about you, and your specific goals for wellness. A physical exam will be done, and appropriate tests will be ordered. We will also make sure you are up to date with all recommended health screenings.
Minor Labs and Procedures
For example, minor suturing, cryotherapy, and skin biopsy are all included in your membership fee. If there is an extra charge for a service, you will know exactly what it is before proceeding. There are special labs and procedures that may require an additional fee. Our members receive a discounted price for these special labs and procedures.
Consultation with Specialists
If you require a consultant for specialty medical care, Dr. Hoffman will work closely with that specialist. The choice of specialty care provider is made together with you and Dr. Hoffman.

NEW SERVICES

We now offer discounted cardiology services such as echo, exercise stress test, nuclear stress test, and Holter monitors as well as discounted pathology for tissue analysis of lesion removal and discounted GI/colorectal services including colonoscopy and upper endoscopy with or without biopsies/polypectomies. IN-OFFICE PROCEDURES

Dr. Hoffman performs a number of in-office procedures. Below is a list of some of the procedures she offers:

  • Annual Wellness Exams
  • EKG
  • Venipuncture
  • Pulse Oximetry
  • Peak Flows
  • Nebulizer Treatments
  • Joint Injections
  • Small Lesion/Wart Removal
  • PAP Smears (pathology fee may apply)
  • Suturing Simple Lacerations
  • Trigger Point Injections
  • Ear Irrigation

Other procedures are available and offered for a small added cost. Additional expense may also be incurred if supplies such as splints, cast shoes or crutches are needed. There will be also be pathology costs associated with sending skin biopsies to the laboratory for further testing, and if other outside tests such as X-rays or ultrasounds are needed. These added costs are always discussed with patients prior to ordering any tests or treatments.

OUR RESPONSIBILITY TO PATIENTS

Primary care medicine is the foundation of good health and wellness, and a close partnership with the individual who provides primary care improves patients’ chances at wellness. The current healthcare system does not always emphasize wellness, and certainly does not promote individual partnership/relationships between patients and the persons who care for them.

Vanguard Direct Family Medicine is different. Our goal is to keep you healthy by restoring the sanctity of the provider-patient relationship. We have the time necessary to get to know you, identify your health needs and develop individualized wellness plans and appropriate treatment options that are mutually agreeable. Dr. Hoffman looks forward to getting to know you and is grateful that you would place your trust in her to care for you and your family.

Additional charges may apply for procedures, tests, labs, etc. Those costs will be discussed with the patient before administration.
